Provided below is a PDF version of our Case History Form used to assess patients before treatment begins. Please feel free to download this form and complete it before your initial visit. Any questions you may have regarding the Case History will be gladly answered upon your arrival.
If you were involved in a motor vehicle accident please also fill in our ICBC form.
New Patients
New patients are always welcome. As we have not met you before, we need some pertinent information to allow us to help you achieve optimum health.
One of our therapists will be happy to discuss your need for treatment to determine how long your first appointment should be.
Are you coming to see us under an ICBC, MSP, RCMP or Veteran's Affairs claim? If so, please let us know and have your claim number and information available.
We will inform you of our fees, policies, location, schedule availability and answer any questions you may have. We will also ask you to electronically fill in a case history prior to your appointment so that we are prepared for your visit.
